Submit your information below to request psychology or career coaching services. For both service lines, an initial phone screen is available by request (under 15 minutes to assess goodness-of-fit and provided free of charge). For psychology services, the first appointment is an evaluation and treatment planning session (50-60 minutes and billed to insurance or a credit card on-file if you are out-of-network/self-paying). A co-payment, coinsurance, or payment toward your deductible may be due for individuals using their insurance for psychology services. Payment is due at the time of service or upon clinician receipt of the insurance payment report detailing patient payment responsibility. To maximize privacy, please only include your first name, last initial, the email address and phone number where you consent to receive communications, as well as a brief summary of how we may assist you.
